The FishBone Diagram is another method commonly used for more complex problems where a nonconformity can have different categories of causes that contribute to the problem. The reply to this question can lead to at least two different pathways: due to an issue with the printing machine or because the ingrediente was’t introduced in the product list. As the name implies the method consists in asking 5 times why something happened, looking to move away from the symptoms and finding what REALLY caused the problem.įor example, if an ingredient is missing in a product label the first why should be why is it missing. The 5 whys is an interrogative technique aiming to find the root causes of nonconformities (yes, there can be more than one cause for a nonconformity!). I would like to share with you two common methods used to perform root cause analysis. This means that every food safety professional should be able to perform cause analysis when a nonconformity is raised. In a note of this definition, ISO goes further and mentions that Corrective action includes cause analysis.
